You must obtain or complete a LPA National Vendor Declaration (NVD) for every animal that is introduced to and leaves your property. This NVD is your guarantee that your on-farm practices meet LPA requirements.

Note: Keep records for a minimum of 3 years, in accordance with State legislation or for the duration of the livestock on the PIC, whichever is longer.

Do not amend the pre-printed PIC. Each NVD can ONLY be used for livestock located on the printed PIC.

  • Owners name
  • Property Identification Code (PIC)
  • Address of property
  • Number of livestock being moved
  • Year born (Sheep and Lamb and Goat LPA NVD)
  • Description, including breed, sex, type
  • Other PICs on Ear Tags (Sheep and Lamb LPA NVD only)
  • Brands and earmarks

Number of electronic NLIS devices in the consignment (how many animals have a eID).

Name and address of the consignee. Include the destination address if different to the consignee details. When selling at a saleyards the consignee will be the agent and the destination (and PIC) will be the saleyards. 

The destintation PIC is required in WA and TAS. Contact the receiver, agent or saleyards to find out correct destination PIC. 

Only tick Yes or No to questions, do not tick both. If some of the consignment require you to tick Yes then this applies to the entire consignment.

Read and answer all questions and record details of any chemical treatments within a withholding period or export slaughter interval.

Do not send livestock to slaughter if they are still within Withholding Periods (WHP) or Export Slaughter Intervals (ESI). Information about ESI and WHP for all livestock species can be found at www.apvma.gov.au/ESI

Must only be completed by the owner or person responsible for the husbandry of the livestock. Your signature verifies that the livestock described on the LPA NVD meets the LPA program requirements. An unsigned LPA NVD is invalid. 

  • Date and time of truck departure
  • Registration number of vehicle 
  • Name and signature of person in charge of livestock being moved
